The BWS&A Incident Investigation Course is designed
to focus on identification of cause in order to prevent
reoccurance.
The Incident Investigation course is useful to all persons
involved in incident investigation or incident prevention. The
course walks the student through each step of the incident
investigation, incident analysis and root cause of
identification. This course also teaches attendees how to use
what is learned during incident investigation to prevent
reoccurrence.

The course has been developed in such a way, as to help prepare
the attendee for the International Society of Mine Safety
Professionals examinations.
The following outline depicts much of the materials focus.
Which Incidents Require Investigation.
This section teaches the attendee what an incident is, when it
should be investigated, and why.
Incident Investigation Tools. This
section teaches the student what tools are needed to properly
investigate, collect evidence and preserve the findings.
Incident Investigation Procedures. This
section teaches the attendee how to handle MSHA/OSHA during
investigations, while at the same time protecting the company. It
teaches the attendee how to develop procedures for collecting facts
and how to put the facts in a logical order. This section also
teaches the attendee the value of obtaining facts over opinions.
Incident Reporting. This section
assists the attendee with learning what incidents must be reported
and when they must be reported. It also teaches the attendee how to
fill out MSHA reports in a manner to reduce liability.
Report Writing. This section
instructs the attendee how to write reports. It also teaches four
generation reporting for all serious injury or fatal incidents.
Investigation Responsibilities. This
section covers who is responsible for doing what, when. It also
covers how to make up a safety committee for incident investigation.
Communication. This section
instructs the attendee how to communicate incidents, and how to
distribute reports and bulletins.
Root Cause Analysis. This section
provides the student with a method of determining root cause. It is
a hands-on opportunity for the student to work on actual incidents
that have occurred, and develop remedial action.
Witness Interviewing. This section
is designed to teach the safety professional how to interview
witnesses and others to obtain factual information.
Incident Reconstruction. This
section will explain how to reconstruct an incident, and how to
apply laws of science to reverse incident events.
